3. Transcript of Records
4. Diploma
5. Photocopy of Certificate of Eligibility/PRC license /Driver’s license
6. Rated and Approved IPCR JANUARY – JUNE 2020 (Received by MPDO)
7. Certificate of Trainings attended (within the last 5 years)Deadline for submission of application portfolio is on AUGUST 7, 2020.
